I currently accept cash pay and the following insurances:
- Regence/Blue Cross Blue Shield commercial plans
- MODA commercial plans
- Pacific Source commercial plans.
I do not accept Medicaid or Medicare.
For cash pay service and insurance that I am not in contract with, I can provide you with a superbill, which is an itemized invoice you can submit to your insurance company for reimbursement. As an out-of-network provider, your insurance may reimburse you based on their out-of-network coverage policies. Please note that you will be responsible for any difference between the fee clinic charged and the amount reimbursed by your insurance.
I encourage you to contact your insurance company directly to inquire about their out-of-network reimbursement policies and procedures.
