Steps to request services:
If you're a new patient, welcome! I am currently only providing virtual visits in Oregon. Here’s how to request service:
- Select a "New Patient Visit" appointment and fill in the basic demographic information.
- Choose a time. This will NOT confirm an appointment—it just holds a spot so you can complete a brief inquiry form to share your concerns, needs, and some background.
- Fill out the inquiry form
- You’ll be asked to enter your credit card info. You will NOT be charged at this stage. I know it may feel unusual to do this before being accepted into the practice, but this is how my electronic medical record system (CharmHealth) is set up. It uses Bluefin, a secure payment processor that allows you to enter your card info yourself, rather than having me do it. I can only see the last four digits and expiration date—everything else stays private between you and Bluefin.
- After I review your form and determine if my practice is a good fit, I’ll send you an invitation to create a CharmHealth account.
- Once your account is set up, I’ll send you secure messages through the portal with intake paperwork and scheduling instructions.
