Steps to request services:
If you're a new patient, welcome! I am currently only providing virtual visits in Oregon, and only accept Blue Cross Blue Shield insurance or cash pay.
To inquire about becoming a new patient, you must follow the steps below. This is the only way to request services—I am not able to respond to phone calls or voicemail inquiries.
HOW TO GET STARTED:
- Select an "Telehealth New Patient Visit", and fill in the basic demographic information.
- Choose a time. This does NOT guarantee an appointment, service, or establish patient-provider relationship. It simply acts as a place holder so that you can fill out a brief inquiry form via a secure access platform where you can share your concerns, needs, and some background.
- Fill out and submit the inquiry form as instructed.
- I will review your form within 5 business days. If it seems like my practice is a good fit, and I have room for new patient, I’ll send you an invitation to create a CharmHealth account.
- Once your account is created, you will see some questionnaires and consent forms in your patient portal. Please review my practice policies/consent forms carefully.
- If you agree to the policies listed in the forms and want to proceed by requesting appointment, please fill out the questionnaires and sign the consent forms.
- You can then request an intake appointment within your patient portal.
- You may also request appointment via my website by: selecting service type "Telehealth New Patient Visit (70 mins" --> Select YES under question "Are you an existing patient" and enter your demographic information.
