Steps to request services:
If you're a new patient, welcome! I am currently only providing virtual visits in Oregon.
I currently accept cash pay and the following insurances.
- Blue Cross Blue Shield (BCBC) or Regence
- MODA
- Pacific Source
I am out of network for all other insurances including Medicaid and Medicare, which means I will not be able to bill the insurance but can provide you with a superbill. You can present the superbill to your insurance company for possible reimbursement.
To inquire about becoming a new patient, please follow the steps below. This is the only way to request services—I am not able to respond to phone calls or voicemail inquiries.
HOW TO GET STARTED:
- Select an "Telehealth New Patient Visit" and enter your demographic information
- Choose a time. This does NOT guarantee an appointment, service, or establish patient-provider relationship. It simply acts as a place holder so that you can fill out a brief inquiry form via a secure access platform where you can share your concerns, needs, and some background information.
- Follow the prompts from the online form to answer questions and submit the inquiry.
- I will review your form within 5 business days and respond by either confirm or deny your request. If it seems like my practice is a good fit, and I have room for new patient, I’ll confirm your appointment request and send you an invitation to create a CharmHealth account which will provide a HIPPA compliant and secure channel for our communications.
- Once your account is created, you will be able to access required questionnaires, clinic policy and consent forms in your patient portal. Please review my practice policies/consent forms thoroughly.
- If you agree to the policies listed in the forms and want to proceed by requesting an appointment, please fill out the questionnaires and sign the consent forms at least 5 days prior to your scheduled appointment. Your appointment is subject to cancellation if required forms are not received on time.
For established patients (those who have already created a portal account), you may request appointment directly either through your patient portal or through my website. Please note that once your portal is created, you are considered an “existing patient" when scheduling.
